		Better Health The Better Health Team is a multidisciplinary team committed to improving the health of the Moreland community. The team includes occupational therapists, community health nurse, a diabetes nurse educator, dietician and speech pathologist. >
		Case Management Services Case Management Services operate across the municipalities of Moreland and Hume and provide support to frail older people, people with dementia, younger people with disabilities who have complex and/or changing care needs and their carers. >
		Children's Health The Children's Health Unit includes specialties in audiology, occupational therapy, paediatrics, speech pathology, dietetics, physiotherapy and podiatry. >
		CoCare CoCare the Youth health team at Moreland Community Health Service. The team provides counselling, group work, advocacy and support to young people aged 12-25 who live, work or study in the City of Moreland. >
		Commonwealth Respite and Carelink Centre / CarerLinks North (CRCC/CLN) The Commonwealth Respite & Carelink Centre / CarerLinks North provide a range of free services to carers of people who are aged, disabled or chronically ill. The focus of our service is supporting carers in their caring role. >
		Dental Service Our Dental Team provide quality general and specialist dental services for people on health care or concession cards and young people between 12-18 years old. >
		Duty Service Duty Workers are available for a set time each day to provide information and assistance to the people who wish to access social welfare services. The Telephone Interpreter Service can be used for people who speak a language other than English. >
		Exchange / Avalon The Exchange and Avalon Outreach Programs provide services for people who have a primary diagnosis of a mental illness. >
		Family Support The Family Support Program offers individual, couples and family counselling; in-home support; developing routines; discussing parenting issues; parenting and developmental groups; mediation between adolescents and their parent(s); support to families experiencing major life crises eg. divorce/separation, death in the family and support to families experiencing major life changes eg. birth of a child, blended families.
		Foot Care The Podiatry Service provides diagnosis and treatment of foot and lower leg disorders. The podiatrists provide foot care services to people of all ages and also work with local groups to ensure the local community is educated about foot health. >
		Foundations Early Childhood Intervention
		Fun 'n' Healthy in Moreland Fun 'n' Healthy in Moreland is part of the new national push towards healthy living for children and is an opportunity for schools, families and local community organisations to get involved in a range of activities designed to increase physical activity, healthy eating and positive self esteem. >
		GEL (Girls Experience Life) Group GEL is a support group for Mums with a mental illness. This group offers an opportunity for Mums to get together to provide support and friendship to each other, get parenting tips, engage in self-care activities, explore ways to build confidence and self-esteem and activities together with children in the school holidays. 		Health Promotion Health Promotion supports the notion that being healthy isn’t just about ‘not being sick’ or physically unwell, it takes a holistic view towards health, acknowledging that good health involves supportive environments, and social and emotional factors that affect health and well-being. 		Medical Brunswick Community Medical Centre (BCMC) is a family medical practice, co-located with Moreland Community Health Service (Brunswick). BCMC offers a comprehensive GP service, including – Acupuncture, Cryotherapy, Immunisations, Women’s Health, Minor Surgical Procedures, Pathology Collection, Psychologist and Psychiatrist and Recall / Reminders. >
		Melbourne Easy Breathers Easy Breathers is a pulmonary rehabilitation program for people with breathing problems. It's a comprehensive program of exercise and education designed to improve physical fitness, reduce breathlessness and provide people with chronic lung disease with a better understanding of their disease and how to best manage its symptoms. >
		Multicultural Counselling & Welfare Social workers offer short term counselling and support to individuals. This can be for a range of issues, such as family problems, grief, loss and relationship difficulties. Where appropriate, we provide referrals to other agencies. Our services are culturally sensitive and respectful of individual needs and circumstances. >
		Needle & Syringe Program A Needle Syringe Program runs between 9am and 5pm from Monday to Friday at the Coburg & Brunswick sites. >
		Occupational Therapy Occupational therapy services assist frail aged and disabled people to stay in their homes safely. Our Occupational therapists provide assessment in people's homes and assist in the provision of appropriate equipment to make day to day living easier and safer. >
		Physiotherapy The focus of all Moreland Community Health Service physiotherapy services is to teach clients how to manage their own health problems, so as to reduce disability and encourage well being. >
		Planned Activity Groups MCHS provides 15 Planned Activity Groups (PAGs) a week, funded through Home and Community Care for frail aged and people with disabilities. The programs promote social involvement, companionship and carer respite within a supportive environment. PAG operate from a number of facilities within Moreland. >
		Raft Raft is a program for families whose lives have been disrupted due to drugs and alcohol. We offer counselling for parents, partners and children, support with developing strategies to deal with the drug problem and support with addressing issues that undermine family relationships. >
		Victims Assistance and Counselling Program The Victims Assistance and Counselling Program assists victims, their families, or anyone affected by crime. By providing support counselling services, outreach services, support groups, volunteer assistance, advocacy, assistance to make a formal report to the police, support during legal processes and more. >
